What is the Insurance Marketplace?

Commonly known as "Obamacare"...

The Health Insurance Marketplace is designed to make buying health coverage easier and more affordable. The Marketplace allows individuals to compare health plans, get answers to questions, find out if they are eligible for tax credits for private insurance or health programs like the Children’s Health Insurance Program (CHIP), and enroll in a health plan that meets their needs.

The next open enrollment period begins November 1, 2016 – January 31, 2017 for coverage that would start as early as January 1, 2017.

Coverage Options outside Open Enrollment

Outside the normal open enrollment period, you can enroll in a private health insurance plan through the Marketplace only in limited, exceptional cases, or if you have a qualifying life event like getting married, having a baby or moving outside of your insurer’s coverage area. Medicaid (Tenncare) and CHIP enrollment also continues year round.

If you have questions or need help, we have Certified Application Counselors at each clinic location to help.


Q. What should I expect on my first visit as a new patient?

Welcome! Thank you for choosing us as your provider.  We appreciate the opportunity to take care of you and your family.  Our staff is dedicated to helping you maintain good health by providing you with quality care. 

To ensure your first visit with us is a pleasant one, please review and complete the following:

  • Complete all registration forms including signatures
  • Arrive 30 minutes early for your appointment
  • Prior to appointment, obtain and send previous medical records, lab results or immunization
  • Bring ALL current medication bottles to the appointment
  • Bring insurance card(s) to your appointment. Patients without insurance cards will be considered self pay and may qualify for a sliding fee discount.
  • Self pay patients should complete attached sliding fee forms and bring financial documentation. Accepted forms include any verification of assistance (Food Stamp, Unemployment, Disability, etc…), recent bank statement, check stub, tax return or W2.
  • Bring a photo ID; Federal Regulations require that you provide a photo ID.
  • As a courtesy to our patients and staff with allergies, please refrain from wearing cologne or perfume during your visit.
  • Our clinics are Smoke Free. No smoking except in designated areas.

All patients are responsible for co-pays, deductibles, coinsurance and charges for services rendered at the time of their appointment.

Should you arrive more than 15 minutes past your scheduled appointment time, you may be asked to reschedule for the next available appointment time.

Please call if you need to cancel and/or reschedule your appointment.

Q. How do I get my prescription refilled?

If you are a current patient and are in need of a prescription refill, call our office at the clinic location where you are a patient or use the patient portal.   It may take 24 to 48 hours to process your refill, depending on the time of day and part of the week the refill request is received.   For best results, provide the name of the medication and the name of the pharmacy you prefer.

If you are calling for a prescription refill after office hours, the after hours answer service cannot approve any prescription refills, please contact the office the next business day.

Please remember there will be NO narcotics or other controlled substances refilled by phone.   To obtain refills for narcotics or other controlled substances the patient must make an appointment and be seen by their provider.   (This is according to current State and Federal Guidelines and Regulations).

Q. What if I am having an emergency?

If you are experiencing a life-threatening emergency, please call 911.

If you are unsure if you are experiencing a true emergency, and it is during office hours, please call the office.

If you call after office hours you will be directed to the answering service. Wait for the prompt to speak with a healthcare provider.

Q: What if I don't have insurance?

No insurance? No problem. We offer a sliding fee discount based on household income. The sliding fee covers your visit with a doctor or medical provider of your choice and basic labs and X-rays that are performed on site. To qualify, bring a form of proof of income, such as your most current tax return, bank statement, check stub, food stamp card or documents showing the amount of assistance you receive, or income from disability, social security or unemployment. In addition, please bring a government issued photo ID. If you do not have these items at the time of your first visit, you will need to provide them as soon as possible to prevent being billed.

Q: Do you accept insurance?

Yes, we accept most types of insurance for adults and children, and that includes Medicare and Tenncare.

Please bring a Photo ID and your insurance card(s) with you to each visit and notify us when any changes occur with your coverage.   We will file all insurance claims directly with your insurer or we can provide any information directly to you for your insurance company to process.

Insurance co-pays and any account balances will be collected at the time of your appointment.   Three Rivers Community Health – Hickman and Perry County Medical Center, Inc. accepts the following forms of payment for service:

  • Cash
  • Personal Check
  • MasterCard
  • Visa

We accept most insurance types, including Tenncare and Medicare.   We offer a sliding fee discount to patients without insurance and patients who are under-insured may qualify for the sliding fee based on household size and income.

Q: Is this a free clinic?

Perry County Medical Center and Three Rivers Community Health are not free clinics. If you do not have insurance, you may qualify for a sliding fee discount based on household income. Bring some form of proof of income such as the most recent tax return, bank statement, check stub, or a letter stating the amount of disability, social security, unemployment or other benefits or a food stamp card. In addition, bring a government issued photo ID such as a driver's license.

Q: What do I need to bring to my appointment?

If you are a new patient, bring 1) Photo ID and 2) proof of insurance. If you do not have insurance, in addition to the photo ID, you will need to bring proof of household income and be prepared to pay a fee for service (the cost of your visit may be reduced based on a sliding scale which will be determined by your household income). Proof of income includes one of the following: most recent tax return, W2, bank statement, check stub or a letter stating the amount of disability, social security, unemployment, a food stamp card. If you do not have these types you can provide a letter of support from your employer or an individual who provides some type of support.

If you are an established patient, please bring 1) photo ID  2) Proof of Insurance and 3) Co-Pay

Children under age 18 must be accompanied by a parent/legal guardian or other legally responsible party to all appointments. Also, please note that if you are bringing your child to a well child appointment, do not forget your child's shot record.  The doctor may not be able to complete the school form if you don't bring the shot record.

Q: Do you take "walk ins" or can I make a same day appointment?

Perry County Medical Center in Linden and Three Rivers Community Health - Hickman does accept walk ins on a first come first serve basis.

Both locations will make same day appointments if you prefer a confirmed appointment.

Each health center has its own appointment line and a specific time in which the line opens in the morning. To schedule an appointment, you will need to call the appointment line of the health center you would like to visit the morning you want an appointment.  Appointments are given on a first called, first served basis, and fill quickly each day. Please call at the time the appointment line opens in the morning.  If all slots are already filled for the day by the time you call, you can be placed on a wait-list in case someone cancels their appointment.
