Chronic Disease

Just because you have a chronic disease doesn’t mean you have to give up on life. We give you hope for the future and the assurance that you can maintain a good quality of life despite your condition.

When chronic disease is a principal medical problem it is critical that the patient take an active role to ensure proper management of chronic conditions like diabetes, cardiovascular disease, asthma, rheumatoid arthritis, osteoporosis, COPD. Management and resources are directed by the patient’s primary care physician. Our medical providers work with patients to control their disease, manage symptoms and reduce risk of complications.  Ongoing support will motivate and encourage patients to promote positive outcomes.

 

Diabetes Care Center

Providing comprehensive support in preventing and managing Type 1 and Type 2 diabetes includes screening all patients for pre-diabetes using a quick, non-fasting hemoglobin A1c test that can be done as part of a routine office visit. Identifying pre-diabetes is an opportunity for patients to improve their health and prevent progression from pre-diabetes to type 2 Diabetes.

As a recipient of the State of Tennessee Department of Health Project Diabetes Grant, Perry County Medical Center and Three Rivers Community Health established Diabetes Care Centers to support diabetic patients who want to reach their self-management goals. At the Diabetes Care Center we provide education and support for making permanent lifestyle changes in diet, exercise and weight loss as a way to slow the progression of the disease and in some cases bring blood glucose levels back to normal. The diabetes care team includes individual Behavior Health counseling and group classes, and instruction in how to effectively self monitor blood glucose and injects insulin. This program helps patients build a strong foundation of self-management and self-help.
