Jeffry Smith
Dr. Smith is the Chief Medical Officer and a physician at Three Rivers Community Health-Hickman in Lyles, TN. He received his Medical Degree from Meharry Medical College in Nashville, TN, in 1990.
Dr. Smith completed his residency training at the East Tennessee State University Kingsport Family Practice Residency Program in Kingsport, TN, where he served as Chief Resident. Prior to joining the staff at Perry County Medical Center in 2010, he served on the medical staff at CaroMont Family Medicine in Kings Mountain, North Carolina.

I’m from Linden and have lived here my entire life. I earned my BSN from the University of Tennessee at Martin and later completed my MSN at Arkansas State. I’ve been an RN for almost 5 years, but my nursing roots go back to working as a CNA in high school and throughout college.
My nursing experience ranges from emergency medicine to long-term care — complete opposites, but both have shaped me into the provider I am today. I truly love nursing and caring for people, and I’m excited to take the next step in my career as a nurse practitioner. Stepping into primary care feels like the perfect fit for me. I’m looking forward to building long-term relationships with my patients and focusing on preventative, holistic care.
When I’m not working, you can usually find me with my family or tucked away with a good book. I’m grateful for the chance to serve the community that raised me and am excited for what’s ahead.
