Background

Perry County Medical Center has provided excellent medical care in Perry County and Hickman County, Tennessee, since 1979.

Incorporated as a not-for-profit corporation, the Center received funding through a grant from a Rural Health Initiative grant to establish Perry County Medical Center. Shortly thereafter, Perry County funded a new 7,200-square-foot building. Over the years, it has continued to grow and provide quality health services to anyone seeking medical care at our facility, regardless of their ability to pay. A few years later, a modern facility was added in the Lyles community of Hickman County.

Telemedicine

With most homes in our area having access to high-speed internet, people don't always have to travel to the doctor for medical services. Telehealth, or telemedicine, allows doctors or nurse practitioners to diagnose illnesses remotely without patients having to visit a health center. Patients can access telehealth services through a variety of methods, including video conferencing, phone calls, and text messaging. Additionally, telehealth visits are usually shorter than traditional office visits, making them more convenient for both the patient and the healthcare team.

On-site Diagnostic Labs

With our on-site diagnostics, our medical professionals can quickly detect and diagnose conditions without sending samples to a laboratory. This saves time and money for both the patient and the provider.

Affordable Dental Care

A critical need for areas with many low and moderate-income individuals is dental care. Dental care is a very important part of overall health, especially heart health. Our staff has extensive experience treating patients with neglected dental care.

Phillip Tatuim

Phillip Tatum, CEO

Dr. Phillip Tatum is the CEO of Perry County Medical Center, also known as 3 Rivers Community Medical Center, in Perry and Hickman Counties.

Dr. Tatum has been professionally trained and certified as a Nurse Practitioner since July 2006.

Jeffrey Smith

Dr. Jeffrey Smith, Chief Medical Doctor

Dr. Smith is the Chief Medical Officer and a physician at Three Rivers Community Health-Hickman in Lyles, TN. He is responsible for supervising and monitoring all nurse practitioners. 

Cameron Baker

Dr. Cameron Baker, Dental Director

Dr. Cameron Baker currently serves as the Dental Director at Three Rivers Community Health Group, where he provides comprehensive dental services including preventive care, restorative treatment, extractions, and patient education. He enjoys helping patients feel comfortable during their visits and values the strong, long-term relationships built with those he serves.

Kirstie Allen

Kirstie Allen, Insurance Adviser, HR, and Administrative Assistant

Kirstie Allen, LPN, serves as the HR Coordinator and Administrative Assistant at Perry County Medical Center. She is also a Certified Application Counselor and can assist with Health Insurance Marketplace coverage and TennCare applications, including completion and submission. Appointments are required.     TPCA proudly serves as the collective voice of our state's community health centers. We represent 29 of Tennessee’s community health centers.
    OSIS is a non-profit technology services organization that provides expert NextGen® Healthcare technology assistance nationwide to Community Health Centers.
    RegLantern not only develops helpful tools for health centers, but our experienced consultants also provide technical assistance for all your health center needs.
    Our Patient Portal allows organizations to provide a single portal for patients to access their healthcare treatment records and billing.
    The National Committee for Quality Assurance (NCQA) exists to improve the quality of health care. We work for better health care, better choices, and better health.
